Incident Report

This form is for parents, guardians, or third parties (such as pool staff or visitors) to report any incidents, safety concerns, or behavioural issues involving a Rainbow Club member, staff, or visitor during club hours. 

All submitted reports are reviewed by Rainbow Club’s management team and will be followed up where appropriate. To ensure we can respond appropriately and take any necessary action, we ask that you provide accurate contact and incident details. 

Note: This form is not for Rainbow Club staff, who must report incidents using the internal staff system.
